From Chicago/Rockford area (Southeast) –
Take I-90W/I 94 W/Kennedy Expy W,
Stay Left to take I-90 W via exit 43B towards O’Hare/Rockford (portions are a toll)
Merge onto US12/US18 W via EXIT 142A on the left towards Madison.
Take CR-D N exit- EXIT 206A-B
Turn Right onto CR_D/Fish Hatchery Rd.
Turn Left onto Emil Street (less than .1 mile)
Turn Left onto Damon Rd. (basically a frontage road)
Dean Therapy Center/ Dean Business office is just past Perkins and Super 8 on the same side. Park in the lot and enter the doors under Dean Therapy Sign – proceed thru doors, conference rooms are straight down hallway on ground level.


From Green Bay area (Northeast)–
Take US-41 South.
Merge onto WI-26 S via EXIT 113 toward Madison/Waupun/Rosendale.
Merge onto US-151 S toward Waupun/Madison
Merge onto I-39 S/I-90 E via EXIT 97A towards Chicago/Milwaukee.
Merge onto US-12/US-18 W via EXIT 142A toward Madison.
Take CR-D N exit- EXIT 206A-B

From Cedar Rapids, IA area (Southwest)
Take US-151 N/IA-13 N
Follow US- 151 N toward Dubuque, crossing into WI.
Merge onto US-12 E/US-14 E/US-151 N/US-18 E/W Beltline Highway.
Take EXIT 259 toward Todd Dr.
Turn Left onto Todd Dr.
Turn Right onto W Beltline Highway/Frontage Road
Dean Therapy Center/Dean Business Office will be on your left, past American TV and Culver’s. Park in the lot and enter the doors under Dean Therapy Sign – proceed thru doors, conference rooms are straight down hallway on ground level.
